Prior to conducting Drug Screening, Sure-Hire Inc. must order Chain of Custody forms for your company. This is a
multi-part form from one of the labs we utilize with issued control numbers, labels and specific information that
must be filled out by the applicant, collection site, laboratory, transport, and M.R.O. A supply will be shipped
directly to your company for your future use. This form insures State and Federal regulations are met at every step.
